Below are the documents you will need to download, print, fill out/sign and upload into your Sports Connect account in order to complete registration.

 

 

 

 

 

 

 

In addition, you will need to submit Residency Requirements by either uploading 3 Proofs of Residency as noted in the Residency Requirements doc below or the School Enrollment Form which is signed by your school (league id# 0405-2024).

 

 

 

 

 

 

If you are a returning family and had the School Enrollment form signed by your school before or have a residency waiver, these are still valid. You can request a copy of either from our player agent so you can upload.
